PROBLEMS...ISSUES WITH SUBMITTING YOUR ONLINE ASSESSMENT?

A copy of the submitted online assessment will be emailed to each party. To receive this emailed, copy, please add noreply@qemailserver.com to your inbox account. If you don't see the emailed copy of the assessment in your inbox, please check your SPAM box.
SUBMIT YOUR ASSESSMENT
Assessment worksheets and blank forms are listed below and should be used when first meeting with a student so that the student understands the focus of the assessment. It is suggested that course instructors/university supervisors/cooperating teachers meet with each student and discuss the student's performance before completing and submitting an assessment. University Supervisors, cooperating teachers and course instructors can have the student sign the worksheet/form to indicate that the assessment has been discussed.
If you are unable to access the online assessment submission page from your school due to security or firewall issues, please download a worksheet for the correct level of field experience and complete it. Please make copies of this form for the university supervisor and student. The original should be forwarded to the Teaching Performance Center.
