This form is for use by Undergraduate Matriculated (UG), Second Degree (SB), PBM or Teacher Certification Program (GC) Students (GC) ONLY. To notify the English Department of changes to your English major and minor, please fill out all the information below. Fields with asterisks (*) are REQUIRED.

This is NOT in lieu of an official request for change of undgraduate major to the Registrar office.
