REQUESTS FOR PROFESSIONAL DEVELOPMENT HOURS & HONORARIUMS

 

PROBLEMS...ISSUES WITH SUBMITTING YOUR ONLINE REQUEST FOR PROFESSIONAL DEVELOPMENT HOURS?

Apply online for your Professional Development Hours (Preprofessional)
 
Three (3) Professional Development Hours are awarded to cooperating teachers who mentor preprofessional field experience students (junior practicum). 

Cooperating teachers are asked to complete the above online survey to request their Professional Development Hours.

The certificate awarding the professional development hours will be mailed at the end of the semester.

If you are unable to access the internet to apply online for your Professional Development Hours for mentoring a Preprofessional Field student, please click on the link below to download a paper copy of the request.

If you are applying for Professional Development Hours for a semester other than the current semester, then you will need to click on the link below, print out a paper copy of the Request form and mail it to the Teaching Performance Center.

Apply for Honorarium & Professional Development Hours - Only available through Paper submission  Click on the link below to print out the Request for Honorarium & PD hours.

Each Request for an Honorarium and Professional Development Hours must be accompanied by a W-9 form from the IRS for tax reporting purposes.  Your request will be returned to you without an attached W-9 form that has been completed and SIGNED.

A $100 honorarium is given to a cooperating teacher who mentors a full-time student teacher/intern.  Professional development hours are also awarded to each cooperating teacher based upon the intern's educational program. 

In order to receive the honorarium and professional development hours, a cooperating teacher must return the Request for Honorarium and Professional Development Hours by October 1 for the Fall Semester and by February 1 for the Spring Semester.  The certificate and honorarium will be mailed to the cooperating teacher's home address.

Honorarium requests will only be honored if received during the academic year (July 1-June 30) in which the inservice teacher acted as a cooperating teacher.
