If I plan to move out of state, where do I send the verification form to?
You will need to submit an out of state's verification form for the office to fill out. In addition, you must provide your student teaching/practicum/clinical information as well as supporting documents (Praxis scores, letter of employment, etc.) to email@example.com. Your verification form will be scanned and emailed to you for your records. The applicant must provide us with a permanent address to mail the hard copy with the university seal.
